چکیده

In this issue of Circulation, Bengur et all report their studies of the acute effects of captopril on the hemodynamics of children with congestive and restrictive cardiomyopathies. Captopril and other orally effective angiotensin converting enzyme (ACE) inhibitors have been studied in several large trials in adults with congestive heart failure,2-4 but relatively little has been published about the effects of these agents in children, especially those with primary myocardial diseases. In pediatric populations, the overwhelming majority of patients with the symptom complex of congestive heart failure have a congenital cardiac defect rather than a cardiomyopathy as the underlying cause of their symptoms. It is not surprising, therefore, that earlier pediatric studies of afterload reducing agents, including ACE inhibitors, have largely been done in patients with heart defects, primarily left-to-right shunt lesions. These studies have shown encouraging improvements in hemodynamics and symptoms in some affected children,5-7 but the ultimate therapy in most children with a heart defect is surgical correction of the defect, not long-term medical management. Renal scarring secondary to vesicourethral reflux is associated with an increased incidence of hypertension, which has been reported in up to 38% of children with reflux nephropathy. In this study the results of captopril test on 20 patients with reflux nephropathy(reflux+Scarring) were assessed within 8 months. For this purpose we studied the plasma renin activity and blood pressure r...

Ambulatory blood pressure monitoring in prepubertal idiopathic dilated cardiomyopathy children

BACKGROUND AND OBJECTIVES: Ambulatory blood pressure monitoring is recommended for several diseases in childhood; however, there are no reports about its use in the monitoring of children with dilated cardiomyopathy so far. We evaluated the pattern of ambulatory blood pressure monitoring in children with dilated cardiomyopathy and correlated it to height, weight, and body mass index. METHODS: P...

Rapid reversal of heart failure in a patient with phaeochromocytoma and catecholamine-induced cardiomyopathy who was treated with captopril.

A patient with a phaeochromocytoma and severe left ventricular heart failure caused by a catecholamine-induced cardiomyopathy is described. The clinical signs of congestive heart failure resolved rapidly on treatment with captopril and myocardial performance became normal within two weeks of medical treatment with captopril for one week and with captopril in combination with phenoxybenzamine fo...

Captopril prevents experimental autoimmune myocarditis.

Captopril, an angiotensin-converting enzyme inhibitor, is widely used in the treatment of a variety of cardiomyopathies, but its effect on autoimmune myocarditis has not been addressed experimentally. We investigated the effect of captopril on myosin-induced experimental autoimmune myocarditis. A/J mice, immunized with syngeneic cardiac myosin, were given 75 mg/L of captopril in their drinking ...

Comparison of aspirin renogram and captopril renogram in the diagnosis of renovascular hypertension

Renal artery stenosis (RAS) is the most common cause of secondary hypertension. Preliminary data indicate that aspirin renography with hippurate may be more sensitive for detection RAS. In this study 20 patients with known or suspected RAS underwent aspirin renography (20 mg/kg orally 1 hour before injection of radiotracer) and captopril renography (50 mg orally) with 99mTc-DTPA. Renal an...
